Learning Outcomes

The student:
- is familiar with the multiple facilities of gerontological service and care and knows how the clients are chosen to them.
- is able to make and update a care and service plan.
- knows the responsibilities of the head nurse, understands the meaning of leadership and multi-professional co-operation.
- can use the key clinical methods of treatment and rehabilitation individually with each patient.
- carries out pharmacotherapy under supervision and follows the effects of pharmacotherapy.
- can apply theory knowledge in practical care settings.
- acts ethically in the care relation and in the work community.
- understands the meaning of supervision and evaluation for his/her own professional development and is motivated to work in nursing.

Student's Workload

Total work load of the course is 189 hours, including practice information and feedback for a total of 4 hours.

Prerequisites / Recommended Optional Courses

Basics of Clinical Nursing, Pharmacotherapy courses, Introduction to Clinical Practice, Gerontological Nursing.

Nursing practice in home operating environments.

Regional Impact

The internship is mainly done in the work communities in your area. This co-operation in working life enables the employment of nurses and nurses areas.

Internationality

This internship is possible in an international exchange and internship. The student's language skills develop as and the student becomes acquainted with the international environment and nursing. This will strengthen international partnership and cooperation. The TOP4 exchange destinations for VAMK students are Germany, the Netherlands, Thailand and France.

Mode of Delivery / Planned Learning Activities and Teaching Methods

Supervised Nursing Practice at home health care and service units 7 cr. (5 weeks).

Assessment Criteria

Pass-evaluation:
The student
- is familiar with the organization of home care and the responsibilities of a head nurse
- understands the importance of multiprofessional team work
- is able to plan and develop gerontologic nursing
- performs clinical procedures and rehabilitation methods individually for each patient
- works ethically in the care relations and in the work community
- understands the meaning of supervision and evaluation for his/her own professional development

Fail:
The student
- does not understand the organization of home care, the responsibilities of a head nurse or the importance of multiprofessional team work.
- don`t understand the duties or responsibilities of the nurse or the importance of management or interprofessionalism
- can’t plan or develop gerontologic nursing.
- is not able to perform clinical procedures or rehabilitation methods individually for each clients nor safe medication.
- does not follow the principles of professional secrecy or the rules and regulations of the organisation
- endangers patient safety
- does not understand the meaning of guidance and supervision for his/her own professional development

Assessment Methods

Pass-evaluation on the clinical practice period and assignments

Work Placement

Nursing practice in home operating environments are homecare, nursing services for the elderly and home hospital.
